Can I integrate patient leads directly into RealTime (RT) via API?
Last updated: October 19, 2026
Context
Some sites use RealTime (RT) as their CTMS and want to know whether patient leads and referrals can be pushed directly into RT via API integration, rather than relying on manual email notifications or other workarounds.
Answer
Yes, API integration with RealTime is supported. Once RealTime provides the necessary API credentials and the connection is established, leads can be pushed into RealTime with their study statuses. A push happens when someone clicks Push CTMS on the patient’s profile, or when the patient reaches a Push to CTMS step in a cadence. Delfa can also turn on instant pushes of interactions and status changes for your organization.
In the meantime, or as a supplementary measure, it is recommended to set up a daily email digest containing links to patients that were prequalified that day. This ensures that no patients are lost in the process and that your team has the necessary context for each patient — particularly in cases where a direct transfer is not possible and a follow-up call is needed.
Regarding known limitations of the current RealTime V2 API, the following endpoints are currently missing and may affect full automation:
SMS Consent: The V2 subjects GET endpoint does not return SMS consent information. Delfa reads SMS consent from the RealTime patient portal instead when it pulls a patient.
Picklist/catalog values: Fields such as medications and medical conditions only support fixed values, and there are currently no catalog endpoints to fetch these programmatically.
If you have feedback on additional API endpoints that would benefit your workflows, we encourage you to share those requests so they can be compiled and submitted to RealTime for consideration.
